Social Services

A social worker has various job duties in the long term/short term care environment. Social services helps coordinate ancillary appointments such as; podiatry, optometry, dental, psychological, and many more. We also complete monthly, quarterly, and annual assessments to ensure psychosocial needs are being met. We initiate advance directives when needed or requested by the resident or healthcare designee. Social Services also coordinates room changes and facilitates any grievances. We also network with other community health care providers to best serve residents after discharge, by coordinating any equipment, outpatient rehabilitation, or home health care needs. We work with each department in the facility to meet the needs of each resident appropriately before, during, and after discharge from the facility.
